Request an Onsite Clinic
Make vaccines more convenient for your team, residents, or community. The Onsite Vaccination Clinic through Care Centrix Pharmacy can provide service throughout the state of Florida, and for a minimum of 15 participants.
Request an on-site vaccination clinic, and our pharmacy team will help coordinate a simple, convenient experience that brings care directly to your location. We recommend submitting your request at least 7 days before your desired date.

After you submit your clinic request, you will receive an email acknowledgement, indicating your request has been received. NOTE: Your clinic is not scheduled until you receive confirmation from our team. Any changes to day, time, vaccinations, and participants must be received at least 48 hours before the scheduled clinic.

Our Onsite Vaccination Clinic Team will send you a confirmation email. In your confirmation email, you will receive a link to share with participants so they can complete the documentation and consent requirements before the clinic. Online completion will expedite the process on the scheduled clinic day. Click for Participant Intake Forms.

Promote your clinic to ensure high attendance! Strategies include posting signs in your building/facility, sending email reminders and registration links, and posting on social media/your website.
